Create a database query for the Authorize a Representative form

To run a query on the database to find which T1013 forms have been efiled:

1. Launch the database.
2. In Client Explorer, select Filter > Advanced > New > Add.
3. In Classic Database, select the Query tab, then right-click and choose Add.
4. Enter Field Name T1013.B[15].
5. Enter Condition as is equal to from the dropdown.
6. Enter Compare to as Number from the dropdown menu, and enter 1 as the number.
7. Select OK and run the query.
